When you schedule your first appointment, you will receive a Welcome Email from a secure Patient Portal, prompting you to create an account. Once the account is created, you can complete your Intake paperwork and electronically sign documents.  You can also sign up for appointment text reminders. Please complete the online paperwork prior to your initial appointment so to not take time away from you session.  Please notify me via email if you are unable to complete the paperwork.
